What it is

Conservative management (observation) is the approach of monitoring a person with Chiari malformation over time without performing surgery. It is the usual approach for people who are asymptomatic or have only mild symptoms and no concerning features such as a syrinx.

Because the natural history of asymptomatic Chiari I is generally benign — few people managed without surgery later need it — observation is appropriate for many. Conservative management involves watching for the development or progression of symptoms, sometimes with periodic imaging (surveillance MRI) to monitor the tonsillar position and check for a syrinx, especially in certain situations. If symptoms develop or progress, or if a concerning finding (like a new or expanding syrinx) appears, the approach can be reconsidered. Conservative management may also include addressing symptoms (such as headache management) without surgery. This approach avoids the risks of surgery in people unlikely to benefit. Understanding conservative management clarifies the common, appropriate non-surgical approach for many people with Chiari.
