Scoliosis (association with Chiari)

What it is

Scoliosis is a sideways (lateral) curvature of the spine. It is commonly associated with Chiari malformation, particularly when a syrinx (a fluid cavity in the spinal cord) is present, and is an especially important association in children.

A syrinx in the spinal cord can affect the nerves and muscles controlling the spine, leading to scoliosis — and a significant proportion of children with a Chiari-associated syrinx develop progressive scoliosis. For this reason, when scoliosis has atypical features (such as a left-sided curve, an unusual pattern, or associated neurological symptoms or back pain), clinicians may look for an underlying cause like Chiari with a syrinx, sometimes finding the Chiari during a scoliosis workup. In some cases, treating the Chiari and syrinx (with decompression) can halt or improve the scoliosis, especially if caught early, though some children also need scoliosis-specific treatment (bracing or surgery). Understanding scoliosis and Chiari clarifies an important association, especially in children, and why atypical scoliosis prompts a search for an underlying cause.
