IDH status (prognostic value)

What it is

IDH status — whether an adult diffuse glioma is IDH-mutant or IDH-wildtype — is one of the strongest prognostic factors for these tumors. IDH-mutant gliomas generally have a substantially better outlook than IDH-wildtype tumors (such as glioblastoma), which are more aggressive.

The discovery of IDH mutations transformed how adult gliomas are understood and classified, in part because IDH status so strongly predicts outcome. IDH-mutant gliomas (astrocytomas and oligodendrogliomas) tend to occur in younger adults, grow more slowly, respond better to treatment, and have a considerably better outlook than IDH-wildtype glioblastoma — so much so that IDH status is one of the first things assessed and is built into the classification. This is why an IDH-mutant grade 4 astrocytoma generally has a better outlook than IDH-wildtype glioblastoma, despite both being grade 4. As always, it is one factor among several (considered with grade, other markers, age, and more), and it influences probabilities at the population level rather than determining any individual's course.
