What it is

Perfusion MRI is an advanced MRI technique that measures blood flow within a tumor. Because aggressive gliomas tend to have increased, abnormal blood vessel growth, higher blood flow can suggest a more aggressive tumor, and perfusion can help distinguish active tumor from treatment-related changes.

Understanding Perfusion MRI

Aggressive gliomas often stimulate the growth of abundant abnormal blood vessels (angiogenesis), increasing blood flow within the tumor. Perfusion MRI measures this blood flow (often reported as relative cerebral blood volume), providing information beyond standard images: areas of high perfusion may indicate more aggressive or higher-grade tumor. Perfusion is also valuable after treatment, where it can help tell apart true tumor progression (which tends to have higher blood flow) from treatment effects such as pseudoprogression or radiation-related changes (which tend to have lower blood flow) — a distinction that standard enhancement alone cannot reliably make. Perfusion is one of several advanced techniques that add information to the standard MRI, helping guide diagnosis and treatment decisions, though results are interpreted alongside other findings.
