Supratotal resection

What it is

Supratotal (supramaximal) resection is the removal of the tumor plus a margin of surrounding brain tissue beyond the visible tumor border. Because diffuse gliomas infiltrate beyond what is visible, this approach aims to remove some of those infiltrating cells, and it is studied for select cases to improve outcomes.

Standard gross total resection removes all visible tumor, but diffuse gliomas extend microscopically into surrounding brain. Supratotal resection goes further, removing an additional margin of brain around the tumor to capture more of these infiltrating cells. Evidence in selected gliomas (for example, certain lower-grade gliomas and some glioblastomas in non-critical locations) suggests this may improve outcomes such as delaying recurrence or extending survival. However, it is only appropriate when the surrounding margin is in non-critical (non-eloquent) areas, since removing functional brain would cause deficits — so it cannot be done when the tumor abuts critical regions.
