Few procedures sound more alarming than being awake during brain surgery. But an awake craniotomy is a carefully designed, well-tolerated technique with a clear purpose: protecting the functions that make you who you are.

Why do it awake? Some gliomas sit near or within areas that control speech, language, or movement — regions that look identical to surrounding brain and can't be reliably identified by imaging alone. By keeping you awake for part of the operation, the surgical team can map these functions in real time. While gently stimulating areas of your brain, they ask you to talk, name pictures, count, or move a hand. If stimulating a spot disrupts your speech or movement, they know to preserve that area. This lets the surgeon remove as much tumor as possible while sparing critical function — something that's often impossible to achieve safely any other way.

Does it hurt? No The brain itself has no pain receptors. The scalp and skull are numbed with local anesthetic, and you're typically sedated and comfortable while the opening is made, then gently awakened for the mapping portion, then often sedated again for closing. You won't feel the brain being operated on.

What it's like During the awake portion, you're awake but calm, with the team talking you through tasks. A neuropsychologist or specialized clinician usually guides the testing. Patients often describe it as strange but not painful, and the team prioritizes your comfort throughout. People are carefully selected for this approach, and you'd be prepared in advance for exactly what to expect.

It's natural to feel anxious about the idea. But it helps to reframe it: an awake craniotomy isn't about enduring something terrible — it's a sophisticated way of letting your own brain guide the surgeon so they can be maximally aggressive against the tumor while protecting your ability to speak, move, and function.
