What hydrocephalus is Hydrocephalus is sometimes called "water on the brain," but the "water" is actually cerebrospinal fluid (CSF) — a clear fluid that normally surrounds and cushions the brain and spinal cord. In hydrocephalus, too much CSF builds up in the fluid-filled spaces of the brain, called the ventricles. This buildup makes the ventricles enlarge and, in most cases, raises the pressure inside the skull, which is what causes symptoms.

Why it happens CSF is constantly produced, circulated, and absorbed in a delicate balance. Hydrocephalus develops when something disrupts that balance — most often a blockage in the fluid pathways, or a problem with the absorption of CSF back into the bloodstream. (Overproduction of CSF is a rare cause.) The specific reason varies widely and depends on age and circumstances.

Who it affects Hydrocephalus occurs across the entire lifespan. It can be present at birth (congenital), often related to conditions like spina bifida or a narrowing in the fluid pathways. It can be acquired later from bleeding, infection, injury, or tumors. And it has a distinct older-adult form called normal pressure hydrocephalus (NPH), which can cause walking, memory, and bladder problems that are sometimes reversible with treatment.

How it's treated Hydrocephalus is treated surgically, because the underlying fluid imbalance usually needs a mechanical solution. The two main approaches are a shunt — a thin tube with a valve that drains excess CSF from the brain to another part of the body where it's absorbed — and an endoscopic third ventriculostomy (ETV), a procedure that creates a new internal pathway for fluid to escape a blockage. The most important thing to understand For most people, hydrocephalus is a lifelong, manageable condition rather than something that is cured once and forgotten.
