  • Vision changes.
  • In babies — a bulging fontanelle or increasing head size.

Over-drainage This means the shunt drains too much CSF, dropping the pressure too low — sometimes especially when the person is upright, because gravity can pull CSF down through the shunt (the "siphon effect"). Over-drainage can cause:

  • Low-pressure (postural) headaches — classically worse when upright (standing or sitting) and better when lying down — a pattern that's the reverse of typical raised-pressure headaches. This positional clue is characteristic of over-drainage.
  • Nausea, dizziness.
  • In some cases, collections of fluid or blood on the surface of the brain (subdural collections), because over-drainage can cause the brain to pull away slightly from the skull, tearing small vessels. This is a more serious consequence that may need its own treatment.
  • Slit-like ventricles on imaging (very small ventricles), sometimes associated with symptoms.

Why posture matters as a clue A helpful distinguishing feature: over-drainage headaches typically worsen when upright and improve when lying down, whereas raised-pressure (under-drainage or malfunction) headaches are often worse when lying down or in the morning. This positional pattern can hint at which problem is occurring — though only your team can confirm it.

How these problems are managed Management depends on the problem and the type of valve:

  • Adjustable (programmable) valves are a major advantage here — the setting can often be changed non-invasively to drain more (for under-drainage) or less (for over-drainage), avoiding surgery.
  • Anti-siphon devices / gravitational valves are special valve features designed to counteract over-drainage when upright; they may be used or added to address siphoning.
  • Subdural collections from over-drainage may need specific treatment, sometimes including adjusting the valve to reduce drainage.
