Normal pressure hydrocephalus (NPH) is one of the few conditions causing walking, thinking, and bladder problems in older adults that may improve with treatment. The main treatment is a shunt, and understanding what it can and can't do helps set realistic, hopeful expectations.
The goal of NPH treatment The aim is to drain excess CSF to relieve the pressure effect on brain regions controlling gait, cognition, and bladder function — ideally improving symptoms, especially walking. Treatment doesn't reverse aging or any coexisting conditions, but for appropriately selected patients, it can meaningfully restore function and quality of life.
Selecting the right patients As covered in the diagnosis article, not everyone with enlarged ventricles and these symptoms will benefit from a shunt. That's why predictive testing — the tap test (large-volume spinal tap) or extended spinal drainage — is so important: it helps identify the people most likely to improve, particularly those whose gait improves temporarily when CSF is removed. Careful selection improves the odds that surgery helps.
The treatment: a shunt The standard treatment is a shunt, usually a ventriculoperitoneal (VP) shunt, which drains excess CSF from a ventricle to the abdomen. Key features in NPH:
- Adjustable (programmable) valves are commonly used, because finding the right drainage level matters and over-drainage is a particular concern in older adults (it can cause complications like fluid collections on the surface of the brain). An adjustable valve lets the team fine-tune drainage over time without surgery.
- The procedure is the same shunt surgery described earlier, performed under general anesthesia.
Realistic outcomes. What can patients expect?
- Gait usually responds best. Improvement in walking is the most common and often most noticeable benefit, and can significantly help independence and reduce falls.
- Cognition and bladder symptoms may improve too, but results are more variable, and tend to be better when treatment occurs earlier, before cognitive decline is advanced.
- Not everyone benefits, and improvement varies in degree. Some people improve dramatically; others modestly; some little. This is why selection and realistic expectations matter.
- Benefits may take some time to appear and can require valve adjustments to optimize.
Risks and considerations Shunt surgery in NPH carries the general risks of shunt placement (infection, malfunction, bleeding, over- or under-drainage), and older adults may have additional health considerations. The team weighs the likely benefit against these risks for each person. Ongoing follow-up — including monitoring for over-drainage and adjusting the valve — is part of NPH care.
The importance of timing Because results are often better earlier in the course, recognizing and evaluating NPH promptly matters. Symptoms that have been present a long time, or advanced cognitive decline, may respond less well. This is part of why raising awareness of NPH — so it's identified rather than dismissed as "just aging" — is so valuable.
A message of cautious hope NPH treatment offers something uncommon in this area of medicine: a real chance of meaningful improvement, particularly in mobility. It's not a cure-all, and not everyone benefits, but for the right patient, a shunt can restore function and independence that might otherwise be lost.
