Holocord syrinx

What it is

A holocord syrinx is an extensive syrinx that spans most or all of the length of the spinal cord. It is a more severe form than a localized syrinx and can cause more widespread effects.

Understanding Holocord syrinx

While many syrinxes are limited to a few spinal levels, a holocord syrinx extends through much of the cord, from the cervical region downward. Because it involves a large portion of the spinal cord, it can produce more widespread symptoms — affecting sensation, strength, and other functions at multiple levels. A holocord syrinx in the setting of Chiari is managed with attention to relieving the underlying CSF obstruction (decompression), which can lead even an extensive syrinx to shrink, though larger syrinxes may be monitored closely. Sometimes a syrinx-specific procedure is considered if decompression alone does not resolve it.
