What it is

Neurogenic scoliosis is a sideways curvature of the spine caused by underlying neurological involvement — in this context, a syrinx affecting the nerves and muscles that control the spine. It is a common and important association in children with Chiari malformation and a syrinx.

Unlike typical (idiopathic) scoliosis, neurogenic scoliosis has an identifiable neurological cause: a syrinx can affect the nerves and muscles supporting the spine, leading to an imbalance that produces a curve. A significant proportion of children with a Chiari-associated syrinx develop progressive scoliosis. Because of this, scoliosis with atypical features — such as a left-sided curve, an unusual pattern, neurological signs, or pain — prompts clinicians to look for an underlying cause like Chiari with a syrinx, sometimes discovering the Chiari during a scoliosis evaluation. In some cases, treating the Chiari and syrinx (with decompression) can halt or improve the scoliosis, especially if caught early, though some children also need scoliosis-specific treatment.
