The flow path is: lateral ventricles → foramina of Monro → third ventricle → cerebral aqueduct → fourth ventricle → outlets (Luschka and Magendie) → subarachnoid space → absorption. A blockage at any point causes fluid to back up upstream, enlarging the ventricles before the blockage — which is why the pattern of enlarged ventricles helps localize the problem. CSF also moves in a pulsatile, back-and-forth manner with each heartbeat and breath, in addition to its net circulation.
