As children grow past infancy, the signs of hydrocephalus change. Once the skull bones have fused, a child's head can no longer simply enlarge to accommodate extra fluid, so symptoms begin to resemble those seen in adults — driven by rising pressure inside the skull. Knowing what to look for helps parents and caregivers respond appropriately.

Why the picture changes with age In older infants and children whose skulls have fused, increased CSF pressure has less room to expand into. The result is symptoms of raised intracranial pressure, plus effects related to the parts of the brain involved. In some younger children, head enlargement may still be a feature; in older children, it generally is not.

Common symptoms in children Hydrocephalus in children can cause:

  • Headaches, classically worse in the morning or when lying down, and sometimes improving after vomiting.
  • Nausea and vomiting, which may be more noticeable in the morning.
  • Vision problems — blurred or double vision, or difficulty with eye movements.
  • Balance, coordination, or walking difficulties.
  • Drowsiness, lethargy, or difficulty concentrating.
  • Behavioral changes or irritability.
  • Decline in school performance or difficulty with attention.
  • Delayed development or loss of previously gained skills, in some cases.
  • Seizures, in some children.
  • In younger children, an enlarging head may still occur.

Red flags that need prompt attention Some signs warrant urgent evaluation, particularly:

  • Severe or rapidly worsening headache, especially with vomiting.
  • Persistent vomiting.
  • Increasing drowsiness or difficulty waking.
  • New vision changes or eyes that deviate.
  • A change in alertness or responsiveness.
  • Seizures.

For a child known to have a shunt or treated hydrocephalus, these signs can indicate a shunt malfunction and should be acted on quickly (covered in detail in the complications articles).

Why children can be tricky to assess Children — especially younger ones — may not be able to describe their symptoms well. Headaches may show up as irritability, clinginess, or behavior changes; vision problems may appear as clumsiness; and fatigue may look like moodiness. This is why changes in a child's usual behavior, energy, or abilities deserve attention, even when the child can't articulate what's wrong.

What to do If your child has persistent or worsening symptoms like these, contact their doctor. For severe headache with vomiting, increasing sleepiness, vision changes, or seizures — especially in a child with a known shunt — seek emergency care. Evaluation typically includes a neurological exam and brain imaging (CT or MRI).

A balanced perspective Headaches and vomiting are extremely common in childhood and usually have ordinary causes. Hydrocephalus is far less common — but the pattern (morning headaches with vomiting, drowsiness, vision changes, or a known shunt) is what raises concern. Trust your knowledge of your child, and don't hesitate to seek evaluation when something is persistently wrong.
