What it is

An astrocytoma, IDH-mutant, CNS WHO grade 2 is a diffuse glioma that carries an IDH mutation, lacks 1p/19q codeletion, and shows lower-grade features. It is the lowest grade of IDH-mutant astrocytoma and is a slower-growing tumor, typically occurring in younger adults.

This tumor grows slowly but is diffuse — meaning it infiltrates into surrounding brain — so it usually cannot be completely removed and tends to recur or progress over years. It often comes to attention through seizures. Although grade 2 is the lowest grade for this tumor, these tumors can transform into higher grades (3 or 4) over time, which is why ongoing monitoring is important. Treatment is individualized and may involve surgery (removing as much as safely possible), followed by observation, radiation, chemotherapy, or — for some grade 2 tumors — newer IDH inhibitor therapy (such as vorasidenib). The outlook is relatively favorable compared with higher-grade gliomas, though these remain serious tumors requiring long-term care.
